Rehabilitation Practitioner I –
Full-time (37.5 hours/week) position available for working for a Peer Operated Agency, in a recovery-focused, rehabilitation program which serves with individuals who have a mental health condition. Candidate will embrace and have a track record of demonstrating true recovery principles, which include but are not limited to: promotion of health, wellness, employment, natural relationships, community inclusion, and inspiring real life recovery goals and reducing dependency. Responsibilities include successful execution of HCBS services with an emphasis on providing unique person-centered services in the community. Crisis management skills are crucial. A successful candidate will have a strong work ethic, possess the ability to think "out of the box", an upbeat, engaging personality, a strong desire to help people reach their potential and dreams and work well with a diverse population. Must be able to work a flexible schedule as it serves the program and customers, including evening and weekend hours, with some holiday hours.
The successful candidate will have well-established computer and technology skills, have extraordinary organizational and time management skills, excel in written documentation including notes and goal-development, and have exceptional communication skills. Additionally, a successful candidate will have the ability, motivation, capacity and confidence to work independently and must demonstrate good judgement and ethics in decision-making processes.
Minimum qualifications: Must be credentialed as a Certified Peer Specialist. Bachelor’s degree in human service field/rehab counseling or related fields preferred. Personal life experiences and other work experiences considered. Must have reliable transportation with clean NYS license.
